Care Partner is a new approach to kidney care, built on the idea that health care is a partnership. It’s the evolution of our old Care Coordination Services, but it goes much deeper. The new name reflects our goal of fostering trust and collaboration between our patients, their families, and the care team.

Now, every Kidney Care Center clinic is part of the Care Partner program. You can sign up easily online or at the clinic to get started.
