An initial visit with your doctor is required to enroll in this program and receive your device.
After enrollment, a Care Coordinator will call you in the next 48 hours to finalize your registrations and answer any further questions.

Take your readings daily! Consistency is key!
You must complete a monthly phone call with your assigned Care Coordinator from phone number (408) 462-6408. Please save this number in your phone.
If you have Blood Pressure Readings that are extraordinarily high or low, a Patient Care Coordinator will call you to review what is happening.

KCC RPM Services extend patient care beyond office visits, fostering collaboration for effective health management. This approach prevents complications, reduces hospitalizations, An educational component provides patients with tips and guidelines for better health condition management. It empowers patients to take control conveniently from their homes with educational support.
During your appointment, you will be dispensed an FDA approved blood pressure monitor, that will record readings at various intervals throughout the day.
A visit to your doctor is necessary to register for the program and obtain your medical device. During this appointment, you will receive guidance on operating the device, accessing your readings, and crucial information about the program. Following your registration, a certified care coordinator will contact you the day after to provide additional details about the program and address any questions you may have.
RPM services are covered by your primary and supplementary insurance carrier.
If your device stops working, each device is equipped with a labeled phone number for technical support.
